Who qualifies
- The applicant must be a resident of Madhya Pradesh.
- The applicant must be a member of an Economically Weaker Family or a Below Poverty Line (BPL) family.
- The applicant must ensure their name is recorded in the family roster and they hold a ration card.
- The intended beneficiary (child) must be aged 0 to 15 years.
- The intended beneficiary (child) must be suffering from one of the identified heart diseases.

Conditions

- The beneficiary or hospital must submit a detailed expenditure report/utilization certificate for the funds spent on the patient's treatment.
- A duplicate or photocopy of the discharge ticket issued after the patient's recovery must be provided.
Who is excluded
The beneficiary is not eligible for assistance if treatment for the sanctioned disease is sought prior to the approval date.
